Please complete the required Screening Form within 3 days of your scheduled. On the day of your appointment, please arrive 10-15 minutes early for additional health screening.

Our waiting room remains closed for now. We will call you when it’s your turn. Upon entering the building, you will be greeted at the main entrance and asked to use hand sanitizer. You will then be directed to your treatment room.
We kindly ask that only patients enter the office.
An exception will be made for young children and anyone requiring assistance. In these cases, only 1 parent or guardian is permitted.
In compliance with CDC guidelines all individuals must wear a mask that covers the mouth, nose, and chin. Bandanas, gaiters, and masks with open valves for exhalation are not allowed. An exception will be made for children less than 2 years of age and those medically unable to wear a mask.
If you or anyone in your household is in quarantine or awaiting test results for any upper respiratory infection, please call to reschedule your appointment. Additionally, all travelers and those who attend gatherings outside their household units are subject to quarantine for 14 days prior to routine visits. Please ask about teledentistry for urgent matters.
Appointments are limited until we are able to operate at full capacity. Priority will be given to patients with emergencies.
